Medical Clearance Information

It is YOUR responsibility to call and schedule your appointment. Please notify our office once this is scheduled so that we can be looking for your clearance note.

Clearances may be scheduled no sooner than 30 days before your surgery and no later than 10 days before surgery. If scheduled more than 30 days before your surgery, your clearance will be invalid. If scheduled less than 10 days out from your surgery, there is a risk of your surgery being cancelled.

  • What to bring:
    • Insurance card, photo ID, list of current medications
    • Copies of any recent blood work or EKG
    • Specialist clearance letter with associated testing results, if applicable

Medical Clearance- Your Primary Care Physician**must be 10-30 days from surgery date

If you go to your Primary Care Physician for your pre-operative clearance, you must remind them to Fax your History and Physical examination document, lab work, and ECG tracing to our office at 708-409-5179. We will not have access to their records, and we need to provide these documents to the surgery center/hospital prior to your procedure.
