Your First Visit

What to Bring to Your Appointment

  • Driver’s License or a valid ID
  • Insurance information
  • Reports, X-rays, MRIs, CT scans, prior operative reports or clinic notes, microbiology and laborator reports etc. and any other relevant information that might help Dr. G get a clearer and more accurate picture of your history and condition
  • List of medications (if any)
  • Important: Dr. G reviews all of his patient’s imaging. The radiology report is not sufficient. Please bring copies of any relevant prior imaging on a CD from the imaging center for us to upload and review.

Where and When

New patients should arrive ** minutes prior to their appointment time to fill out patient forms.

To better serve our patients note that we have multiple locations throughout Chicagoland. Please double check the location and time of your appointment.

To cancel an Appointment

Telephone the office during business hours and allow at least 1 days’ notice so that we can offer your appointment time to patients on our waiting list.

We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time and may cause scheduling delays beyond our control. We apologize if we keep you waiting.
